At the Diabetes Center at Park Plaza Hospital, we are pleased to offer a variety of educational courses. In addition to the courses offered at the Diabetes Center, educational services are provided to inpatients and through individual consultations.
We will need to receive a referral from your physician prior to scheduling you to attend our classes or other educational offerings. Please ask your referring physician to fax his or her completed orders to us at 713.285.1224. Click here to access and print a copy of our patient referral forms online, which you can present to your referring physician at the time of your doctor's appointment.
